Provide intensive in-home, crisis-oriented interventions and case management services to high risk youth and adolescents ages 5-21 with their families, to include those individuals who are at substantial risk of out of home placement, transitioning back home from residential treatment, psychiatric hospitalization, or have been placed in foster care.
Provide primary service in the client’s home, 90% of the time, as identified on the client’s Individualized Service Plans (ISP).
Collaborate with client, family members and other resources to develop interventions, goals and strategies for ISP’s.
Educate client and family members regarding existing community resources, and act as an advocate to connect the client and family members to those resources identified as well as to teach the client/family to advocate for themselves.
Attend and participate in appointments such as Psychological/Psychiatric evaluations, IEP/school meetings, case conferences: CAT/FAPT/Family Team Meetings and court appearances with the client and family as necessary.
